If you are already a patient at Wellness out West please select Regular Appointment.
Please note: if you have not had an appointment within the past 12 months you will require another Initial Appointment to review your health.
If you have had a recent injury or accident and you think your appointment may be subsided by ACC please select ACC Initial or Regular Appointment.
A child appointment is appropriate for any child up to the age of 18 years old, or still attending high school.
